Program Planning Documents

Afterschool programs in Vermont have a culture and practice of sharing ideas and resources with one another. The resources and documents included on this page represent some of the program planning and development tools created by Vermont programs and made available to others for their own modification and use.

These tools can be downloaded and modified for use in your own programs. When using any of these examples, the best protocol is to include a brief acknowledgment of original program or author.

Evaluation Plans

Here are four exemplary program evaluation plans identified from 21C programs in Vermont. Many thanks to the directors of the following programs for their permission to share these documents: Beth Brooks at ENCORE, Nicole Miller at REACH!, Deb Bratton at RNESU-SOAR, and Trish Adams at Westminster Afterschool. These plans are each available to download as a sample for how you might create your own evaluation plan:

Sustainability Plans

We have identified a few sustainability plans from 21C programs in Vermont as examples of how some programs anticipate the decrease in 21C grant funding (from 100% federal funding the first year of the grant to 50% funding by the fourth year). These programs have focused their sustainability efforts in a variety of grants, fundraising, partnerships, sponsorships, school budget allocation, foundations, and reimbursements. Many thanks to the directors of the following programs for their permission to share these documents (which are evolving works in progress): Beth Brooks at ENCORE and Betsy Stacey at BEAMS. These plans are each available to download as a sample for how you might create your own sustainability plan:

Program Planning Forms

  • The ENCORE Program in North Country Schools Supervisory Union developed a program information and planning form that instructors fill out prior to teaching in the afterschool program. file_acrobat ENCORE Program Proposal

Advisory Board Roles and Responsibilities

Wings Community Programs in Southern Vermont (Deerfield Valley Elementary School, Halifax School, Readsboro, Central School, Stamford Elementary School, Twin Valley Middle School, Twin Valley High School, Whitingham Elementary School) has a set of guidelines that clearly outline the roles and responsibilities of their advisory board. file_acrobat Wings Advisory Board Guidelines
